Shipping Labels Printing USPS shipping labels using Avery

In M.O.M. 6.0 I'm looking for a way to print shipping labels onto an Avery 5164 label sheet. Is there a way to do this and customize it so I can choose which label I want it to print to? The 5164 sheet has 2 columns and 3 rows.

If it's not possible to configure this within M.O.M. itself, is there any other program that would work with M.O.M. to allow customized shipping labels?
